1.                   I/We hereby apply to borrow a school netbook for use by my/our son/daughter, named above, throughout the 2011 to 2014 school years.

 

2.                   I/We have read and agree to abide by the “Terms and Conditions of Borrowing a Netbook Computer”.

 

3.                   I/We agree to pay the cost of repair/replacement of the Netbook if this is necessary as a result of careless or deliberate actions by my/our son/daughter. I/We have noted the relevant Replacement or Repair section in the “Terms and Conditions of Borrowing a Netbook Computer”

 

4.                   I/We understand that the Replacement or Repair excess amount will also apply if my/our son/daughter is found to be responsible for damage or loss of another student’s Netbook computer.
